Patient Portal

The Patient Portal allows you to view your medical data, submit statements of intention, appoint representative(s) for yourself, and act on behalf of the persons who have appointed you as their representative.

    We would like to inform you that it is not possible to sign documents in the Patient Portal using smartID. If it is necessary to sign a declaration of intent or a health declaration, please use ID card or mobile ID to log in and use the same tools to give a digital signature.
